Have you seen the shoes they stand in their toes in!? Modalities are a good adjunct. I have the best luck with adjusting the metatarsal/tarsal/ankle and passive motion of the metatarsal joints separately in all ranges. I like axial traction adjustments of the phalanx. Get in-between the metatarsals and hit the trigger points around the the inflamed nerve. Also I don't know if orthotics are recommended without the information from exam. I do know that restoring the transverse arch at the metatarsal/phyalangeal joint is very important, this can be done as an adjustive procedure and re-enforced with a metatarsal pad worn in the shoe.
