Outcome Assessment
By Do you use Outcome Assessment in your practice? Patient improvement, documented with pre- and post- scoring on questionnaires (QAs) like the Neck Disability Index, or the SF-36, are indisputable evidence of medical necessity and patient progress.

What outcome assessments forms do you use in your practice? Do you use more then one? thanks, Scott
I use either the NDI or the Oswestry, along with the SF-36. The 2 together make a formidable pair in documenting patient complaints, as well as describing the impact of it on their lifestyle.
